accountability matters it's the link between those in power making decisions and the interests of all of us whether as voters whether it's service users whether it's residents in a local community when it works well it makes UK government among the best in the world but when it doesn't work we see repeated failures failures in the delivery of services chronic underperformance in how government operates and people's live deeply affected by things that should simply never have happened in this paper we look at three underlying causes for weaknesses in accountability in white or where conventions which date
