in this video we will look at a diagnostic framework helping you in the diagnosis of medial tibial stress [Music] syndrome hi and welcome back to Physio Tudors winter at all from 2017 investigated which features from patient history taking as well as physical assessment would be indicative of medial tibial stress syndrome also known as Shin spins which is one of the most prevalent exercise induced overuse injuries in the lower leg pain usually manifests along the distal third of the posterior medial tibial border and is provoked upon palpation over an area of five consecutive CM the first
characteristic that steers you in the direction of mtss is the presence of exercise induced pain along the distal 2/3 of the medial tibial border so if you meet a runner or Sportsman who describes a painful lower leg mostly on the medial side you are on the right track it's an essential point that the pain is provoked during or after physical activity and gets better when the patient has relative rest pay attention and ask for cramping burning pain over the calf or posterior compartment with without numbness or a pinent needle sensation in the foot and this
could indicate chronic exertional compartment syndrome during your physical assessment you should check whether you can reproduce the patient symptoms by palpating the the distal third of the poster medial border of the tibia in a characteristical area of five or more consecutive cenm recognizable pain is essential if the area is smaller than 5 cm chances are that it could be a tibial stress fracture for example if there are no other visible signs or symptoms such as severe swelling or aemma that could indicate other lower leg pathologies you can pretty accurately make the diagnosis of shin spins
