Hello, friends of the heart! How are you? I am Dr Roberto Yano Cardiologist and Pacemaker Specialist.
You sure know someone who has swollen feet, you know that bread-like foot? That person who has an ankle that, when you squeeze it, leaves a hole? But why does this person have swollen legs?
What are the possible causes of this swelling that affects so many people? Do you know? I will explain to you!

Cause number 1: Weak veins. When our veins are weak, the blood that reaches our legs is backed up and cannot return to the heart properly. Our veins have valves that do not allow blood to return and help the blood return to our heart, but for some reason the valves in the veins become weak and can no longer stop the return of blood that passes through them.
What happens then? More blood will accumulate in the lower regions of your body and your feet will start to swell, then your ankles will swell! In this type of swelling, the person usually wakes up with less swollen legs and as the day goes on, they stay on their feet for a long time and their feet swell and in the late afternoon they will be quite swollen.
So when you get home and put your legs up, the trend is for leg swelling to get better! When the veins are weak, the appearance of varicose veins is common. You must have seen people with swollen legs and full of thick and bulging veins, probably these people have venous insufficiency!
And, to diagnose weak veins, we usually request venous ultrasound of the lower limbs, which is usually the initial exam to make the diagnosis! But, let's go to cause number two, you know what it is: liver failure. Have you ever met someone with cirrhosis?
Leave it here in the comments if you know someone with cirrhosis. Have you seen what the biotype of the person who has Cirrhosis is like? Generally, the cirrhotic has thin arms, a well-enlarged belly and swollen feet!
Why does it happen? When the cirrhotic liver no longer works, protein metabolism is impaired and fluid leakage may occur due to increased pressure; liquid falls into the abdominal cavity, giving that real feeling of water belly. The belly is really full of liquid!
All this pressure from the abdominal cavity makes it difficult for blood to return from the legs to the heart, what happens then? The legs will start to feel swollen; the first thing that will get swollen is the feet. Regarding the screening diagnosis, an abdominal ultrasound associated with some blood tests can help the doctor to start the investigation of the cause of liver failure.
For those who don't know, cirrhosis is the end stage of liver failure. There is stage steatosis grade 1, grade 2, grade 3 and finally cirrhosis. Steatosis is a reversible disease, but cirrhosis is not.

Third cause for swollen feet: kidney failure. The kidneys are responsible for removing much of our dirt and controlling our water and electrolyte balance. Imagine that the kidney that was working 100% now works 30%, what will happen?
The organism will start to accumulate, because it will no longer be able to throw the excess out! When fluid build-up occurs, what is the first region you will feel? It's the feet.
You must know someone who does dialysis. Dialysis is done when the kidneys are no longer able to filter impurities from our blood. Do you know someone who does dialysis?

There is a routine blood test called Creatinine. If she is above 1. 2 mg per dl, it's good to stay tuned!
The higher the Creatinine value, it means your kidney is working less! Fourth cause: heart failure. I couldn't help but mention this cause!
Particularly, this is the cause I see most in the office, on a daily basis. If your heart is weak and you are not able to pump blood throughout your body; If your heart used to work 100%, but now it only works 40%, one of the classic symptoms of heart failure is lower limb edema, which is swelling. This swelling is due to the accumulation of fluid into the third space, which is called the interstitium.
In the clinical consultation itself, it is almost always possible to conclude the diagnosis of heart failure. And to confirm the diagnosis, the most requested exam is the echo cardiogram, which is the ultrasound of the heart. In cases of heart failure, the use of diuretic medications can be helpful in relieving symptoms and decreasing swelling, but diuretics are for people with heart failure, right?
It is common to see people taking diuretics to lose weight! You must know people who take formulas containing diuretics and believe they will lose weight! Do you know someone who does it?
Comment here for me! I've seen people take formulas that mix three types of diuretics, thyroid medication, antidepressants, wake-up medication, sleep medication! Everyone believes what they want to believe!
In addition to diuretics, there are excellent drugs for the treatment of heart failure. Even heart failure is one of the areas of Cardiology with more studies, with a rapid evolution in studies! We can use anything from drugs to implants of resynchronizers or defibrillators for heart failure; even heart transplantation in some cases.
So if you have heart failure, make sure you're getting the proper treatment! Every year there are news, so always check with your doctor and never leave the appointment with doubts! I always ask at the end of the appointment if my patient has any questions or if he wants to ask something!
The patient is the one who should know most about the disease itself! The more he knows about the disease, the more he will want to take care of himself! But, let's go to the fifth cause of swollen feet, what is it, guys?
Obesity. I usually see many obese patients in the office. Often, the obese person complains of edema of the lower limbs.
It really happens! The obese retain more liquid, have more difficulty eliminating excess liquid! The obese heart has to pump more blood to nourish the whole body and the obese are more likely to have venous, kidney, liver and heart failure!
Obese people are more likely to have swollen feet! But, what to do then to deflate? I know the chubby ones will be mad at me, but slimming down can really help and get rid of the bloating once and for all!
